Your First Visit:
For your first office visit, please
bring a complete list of medications you are currently taking and
any recent dental x-rays you may have had taken. We will also need
your current dental insurance information. You will be asked to
complete a patient information sheet, which will include your
medical history. Your first visit will consist of a comprehensive
dental exam, and the formulation of a treatment plan for dental
procedures that are indicated.
Insurance and Billing:
We are contracted with many dental
insurance companies. Because some of these companies offer
different plans, please contact your insurance company to verify our
participation in your plan. As a courtesy, we will bill your
primary and secondary insurance for you, provided we have your
current dental information . Please inform the receptionist any
time your dental insurance changes. We collect co-pays and
deductibles at the time of service. Please remember, insurance
is a contract between you and your insurance company and any
noncovered procedures, or fees above the insurance company’s usual
and customary fee schedule, are the responsibility of the patient.
Prescriptions:
Medication will be prescribed at
your Dentist’s discretion only. Requests for medication or refills
should be made during office hours to enable the Dentist to review
your dental record. Prescriptions will not be given on the
weekends. |
